Centered Family Support Services is proud to offer Coordinated Family Support (CFS), a specialized service designed specifically for adults with developmental disabilities who choose to live with their families. Our program was developed in response to the identified gap in services for adults who prefer to remain in their family home.

CFS is a specialized service designed specifically for adults with developmental disabilities who choose to live with their families. Our program provides comprehensive support to both the individual and their family caregivers, focusing on enhancing quality of life, promoting independence, and strengthening the family unit.
To be eligible for our CFS program, you must be 18 years or older, live with your family, and receive Regional Center services. If you're unsure about your eligibility, please contact us or speak with your Regional Center Service Coordinator.
Unlike many programs that focus on residential or community-based services, our CFS program is built around supporting individuals within their family unit. We provide truly personalized support that adapts to changing needs, comprehensive care coordination, family empowerment resources, enhanced caregiver support, and community connection assistance.
Getting started is simple. First, speak with your Regional Center Service Coordinator about your interest in CFS through Centered Family Support Services. Your coordinator will arrange an IPP team meeting to evaluate how our services can benefit you. Upon referral, our CFS specialist will meet with you and your family to discuss your goals and needs, and together, we'll create a personalized support plan.
CFS services are typically funded through Regional Centers. There is usually no direct cost to eligible individuals and families. During your initial consultation, we'll discuss funding options and help you navigate the process of securing coverage for our services.
We provide services throughout the region. Please contact us to confirm if we serve your specific area. We strive to make our services accessible to as many families as possible and can discuss options for those living outside our primary service area.
